Cardiovascular disease, diabetes and stroke are among the leading causes of death in our communities.  There are many national and local programs that engage faith communities as partners in promoting health in these areas.

Empowered to Serve
Empowered To Serve is an American Heart Association project bringing together a “megacommunity” to join together to help improve the health of at risk communities through the faith based sector. The megacommunity is a group of individuals from the faith based sector, organizations, businesses and government coming together to focus on improving health in multicultural communities. This group will share best practices, develop strategies and ultimately help to drive a “sustainable culture of health” within our nation through guiding individuals to better health and changing the environment in which we live.

Diabetes

Project Power

Project Power is a faith-based program targeting the African American community to provide a foundation for integrating diabetes awareness messages and healthy living tips into the life of the family and church. It engages the church in a variety of year-round activities that provide lessons which improve the heath of those church members living with diabetes, their families and the greater community as well.

Stroke

Power to End Stroke

Power To End Stroke is an education and awareness campaign that embraces and celebrates the culture, energy, creativity and lifestyles of African Americans.

A joint project of Advocate Health Care & the OCEAN-HP at the University of Illinois at Chicago.
